POLICIES AND FEES

APPOINTMENTS

Initial evaluations are scheduled after a brief telephone conversation. They are typically 120-minute sessions and involve obtaining a comprehensive history with the goal of working with you/your family on obtaining an accurate diagnosis and collaborate on creating an individualized treatment plan. Please sign up for the Luminello patient portal and complete and submit the new patient forms at least two business days prior to the appointment.

Psychiatric consultations/second opinions are 120-180 minutes and will involve review of prior records, an extensive diagnostic interview and discussion of treatment, and provision of our clinical impression to the referring provider via written documentation. Please provide prior records to me at least two business days prior to the consultation.

Follow-up visits are generally scheduled at the conclusion of a visit. The timing and frequency of follow-up appointments are determined by clinical need and will be discussed with you at your initial appointment.

All services are available by appointment only. Appointments are currently available in person and via telehealth.

INSURANCE

At this time, I do not participate in any insurance plans. If you have medical insurance, you have the option of filing claims for reimbursement for out-of-network service with your insurance. I will provide you with the necessary documentation (including documentation of payment, diagnostic codes and treatment codes) to submit a claim directly to your insurance carrier.


I have opted out of Medicare. If you have Medicare and wish to receive psychiatric services with me, you will be required to sign a written agreement stating you will not submit claims to Medicare for reimbursement.

FEES

Fee Schedule
Initial Appointment: $875.00
50-Minute Session: $340.00
25-Minute Session: $205.00
Single Consultation: $410.00/hour

Fees are due at the time of the visit and are payable via check, credit card or cash. Please call Dr. Sun to discuss the fee schedule.

If you are late for an appointment, you will be seen for the remaining time in your appointment but the charge will be for the full amount of the scheduled session. If you are unable to make an appointment, please call me as soon as possible and leave a voicemail message. Please provide 48 hours notice if you need to cancel your appointment to avoid fees for late cancellations. Please keep in mind that insurance companies do not provide reimbursement for missed appointments. In the case of inclement weather, please call me and missed appointments will not be subject to missed appointment fees.

Phone calls are generally not subject to additional fees, but you will be encouraged to return for an appointment if your concern cannot be addressed with a brief telephone discussion. In addition, I am happy to complete brief forms/letters such as school or camp medication forms, FMLA paperwork, or prior authorization requests. More extensive documents such as those required for insurance appeals or IEP/504 services will incur a fee commensurate with the time spent.

REFILLS

Prescriptions will be provided at the time of the appointment or sent directly to your pharmacy electronically. If you require a refill between appointments, please call me at 410-881-4639 and leave me a voicemail with the following information: Name, date of birth, callback phone number, pharmacy name/address/phone number, and medication name. Please allow 48 business hours for me to process your request.

CONFIDENTIALITY

I am dedicated to maintaining your confidentiality and right to privacy regarding your health information. You are entitled under federal confidentiality laws to not have release of records or health information without your consent. I will be requesting your consent before contacting other individuals regarding you/your child’s treatment.


There are limits to confidentiality. If there is concern for risk of harm to yourself or others, I will collaborate with you as well as other necessary individuals to pursue appropriate support and treatment.

EMERGENCIES

If you are experiencing a psychiatric emergency, such as active thoughts of harming yourself or others, please call 911 or go to the nearest emergency room. If it is after business hours, please seek emergency services instead of calling the clinic if you have acute safety concerns for you/your child, as I do not check my messages after hours.
